Parents, Student Athletes, Coaches and Doctors

 

Please Read the Following Before You Print Out The Important Forms

  1. A yearly physical exam is required in order for your child (student athlete) to participate in UIL/AISD Athletics.
  2. This physical form must be dated after April 15th and will be valid for one school year.
  3. The History portion of the physical form must be fully completed and answered before the student athlete sees the doctor.
  4. Both the History portion and the Examination/Evaluation portion of the physical form must be signed, dated, and cleared by the physician.
  5. If the physician performing the physical exam does not clear the student athlete and requires further medical evaluation, then the Athletic Trainers at Bowie will need a medical note from that specific specialist clearly stating that the student athlete was further evaluated and is cleared to participate in sports/athletics.
  6. REMEMBER: Only the updated UIL physical form will be accepted . Any other physical forms that are not UIL format will not be accepted.
  7. Also, parents and student athletes are required to complete a “parent and student Agreement/Ackowledgement Form for Anabolic Steroid Use and Random Steroid Testing”.
  8. Parents and student athletes are required to complete a emergency student information card.
  9. The physical, steroid form, and emergency card must be accurately and fully completed and signed by both the parents and student athletes. It must be turned in to the Athletic Trainers only for clearance.
  10. These forms/paperwork are required by the State, UIL, and AISD. They must be accurately completed, signed by the parent and the student athlete, and filed with the Athletic Trainers before the student athlete can participate in any UIL Athletic Sports. (This includes off-season Athletic Classes/Periods)
